Clinical Supervisor, Hospice in collaboration with the Manager –
Clinical Operations, is responsible for the provision of high
quality patient care for hospice clients in one Agency location.
The Clinical Supervisor is responsible for providing high quality
care for assigned patient caseload in addition to supervision of
clinical staff. The Clinical Supervisor serves as the Manager-
Clinical Operations in the absence of the Manager- Clinical
Operations. Essential Duties and Responsibilities Participates with
documentation reviews of clinical staff regarding direct patient
care in partnership with the Manager- Clinical Operations Assist
the Manager- Clinical Operations in the completion of in-home
annual competency and evaluations Facilitates case management
process with staff members, physicians and referral sources
Maintains open lines of communication with all members of the
continuum of care team Conduct ride-along with staff to ensure
patient needs and hospice practices are being met as business needs
arise Assists with educating staff on processes, procedures and
policies as needed Assist with patient care schedules to ensure
continuity and quality regulations Monitor to ensure coordination
and follow-through is complete for all referrals to the hospice
program Assist in the coordination and assignment of all newly
admitted hospice patients to the appropriate team and/or
disciplines Work collaboratively to ensure all admission
documentation is completed within 48-hour period and dispersed
among team members Available to work after-hours to provide support
and/or conduct in-person visits Performs and documents all patient
visits based on assigned patient caseload Reviews visit notes for
content, accuracy, and Medicare criteria Timely submission and
completion of comprehensive assessments within 5 calendar days of
start of episode Participates in team conferences Complete IDG
meeting notes and report on assigned patient case load every 14
days and PRN as meetings are scheduled. Coordinate with the hospice
physician to facilitate and run the IDG meetings in the absence of
and/or in support of the Manager- Clinical Operations. Communicates
with the Manager- Clinical Operations regarding issues requiring
further training and education Attends staff meetings, Company
Medical Group meetings, and in-services as required Follows agency
policies and procedures Responsible to ensure the use of the 4Ms
(What Matters to the patient, Medications, Mentation, and Mobility)
and provides Age-Friendly Care Performs these and all other duties
as assigned by the Administrator and Manager- Clinical Operations

Patients Care Givers Qualifications Required Knowledge, Skills and
Experience A person who is a licensed physician, physical
therapist, speech-language pathologist, occupational therapist,
audiologist, social worker, or a registered nurse. A current
unencumbered State professional license Must maintain a valid
driver’s license and good driving record 3 or more years of nursing
experience Excellent verbal and written communication skills The
ability to assess patient needs and formulate individualized
patient care plans to meet those needs Proficiency in personal
computer use, including e-mail, clinical, word processing, and
spreadsheet software Preferred Knowledge, Skills and Experience Two
years supervisory experience in an in-patient hospital, home care
agency, or residential facility Experience within Hospice Pay
